


Assessment Report Template (Autism/ADHD/Cognitive/Academic)
This report template includes the some of the most commonly used assessments for those working in the area of neurodivergence and/or education: Autism and/or ADHD diagnosis, WISC-V (cognitive) and WIAT-III (academic).
Honed over a decade of practice this report template is all inclusive so that you can simply remove the sections on any assessments you don’t use. It uses affirming language but also includes direct quotes from the DSM-V where appropriate.
While originally designed for clients 6-16 years (due to inclusion of the WISC) it would be appropriate for all ages when using only the Autism & ADHD sections of the report.
